Arrest follows attempt to pass forged prescription

Izard County authorities have made an arrest in connection with an attempt to have a forged prescription filled at a Calico Rock pharmacy.

According to the probable cause affidavit filed Wednesday, 22-year-old Devon Haynes is alleged to have entered the pharmacy Tuesday morning presenting an altered medical prescription slip to the staff. An address for Haynes was not provided on online court records.

Discrepancies in the type of ink on the slip Haynes presented were observed, leading the pharmacy owner to contact the physician who had issued the prescription. The pharmacy owner learned one of the three prescriptions listed for a 60 count of Alprazolam had not been prescribed.

The pharmacy owned notified law enforcement of the forgery attempt, and Haynes was declined service. He left the establishment, with the pharmacy retained the altered prescription form.

Haynes was located by law enforcement at the Izard County Community Medical Center at the time the officer interviewed the physician issuing the prescription.

The officer was approached by a female from Calico Rock who had transported Haynes to the pharmacy. She said Haynes told her prior to arriving at the pharmacy he had been prescribed 60 bars of Xanax. She said she pondered the unlikely event of a doctor prescribing the Xanax for Haynes’ reported health conditions. She said she entered the pharmacy only to hurry Haynes along.

Haynes was charged with a felony count of controlled substances fraudulent practices. His bond was set at $5,000.
